When my doctor recommends therapy, do I have a choice in providers?

Yes, if you need physical therapy, YOU DO HAVE A CHOICE when selecting your provider. We encourage you to explore all your options, and when you do, we're confident that you'll find that ATI will exceed your expectations. With many convenient locations close to home, expert clinicians, and our ability to schedule your first visit within 24-48 hours, we believe that we're the best choice around to help you recover from an injury, surgery or chronic pain.

What can I expect at my first appointment?

Your first visit will last approximately one hour and will include a comprehensive evaluation and applicable treatment administered by a licensed physical therapist. Your physical therapist will review your medical history, diagnostic tests and any recent events that may have contributed to your current condition. Your evaluation will include assessment of your current functional deficits, pain level and posture, as well as a thorough evaluation of your flexibility, strength, balance and endurance.

Following your evaluation, your physical therapist will thoroughly explain your personalized treatment plan and expectations for recovery and provide a commitment letter to you. Communication between the patient and physical therapist is of utmost importance at ATI and you are an active participant in your recovery.

When should I arrive?

Please arrive 15 minutes before your first appointment in order to complete all necessary paperwork.

What should I bring to my first appointment?

  • Photo identification* (Valid Driver’s License, Valid State ID, School ID Card, or Valid Passport)
  • Form of payment due at the time of service
  • Prescription/doctor referral (if applicable)
  • Insurance card (if applicable)

*As a creditor under the Federal Trade Commission's "Red Flags Rule," we adhere to an Identity Theft Prevention Program. For this reason, we ask that you present photo identification at the time of registration. We appreciate your cooperation in assisting us with protecting your identity and the prevention of identity theft.

What should I wear to my appointments?

Comfortable clothing, athletic wear, preferably no jewelry.

What are my healthcare privacy rights?

At ATI, we understand that information about you and your health care is personal. Please view the HIPAA notice of privacy practices to better understand all of your rights.

What will be the duration and frequency of treatment?

Duration and frequency of treatment are unique to each individual’s injury or condition. ATI’s professionals are dedicated to returning patients to optimal health. Our customized programs and individual attention allows patients to recover quickly and enjoy a positive rehabilitation experience.

How much does a treatment plan cost?

The cost of your customized treatment plan depends on your diagnosis, individual needs and insurance plan. We will verify your insurance benefits before beginning treatment to ensure that your care is affordable. To assess your needs, please visit ATI for a complimentary screening.

Who should I contact with billing questions?

We understand that health care bills can be complicated to understand. We are here to work with you to help you understand your bill, get it paid and off your mind. 

Billing-related questions may be directed to the Patient Advocate department at 855-MY-ATIPT (855-692-8478) or email PatientAdvocate@atipt.com.

If the issue is unresolved to your satisfaction, please contact our Patient Dedicated Specialist (PDS) Team at PatientAdvocateEscalations@atipt.com and someone from our team will get back to you.

 If this is a billing dispute, please have your insurance card(s) or billing statements ready. The PDS team will work with the specific clinic where you received services to request a review of charges. We will then contact you with that information.

 If you still have a balance after resolution, our billing specialists will work with you to apply for financial assistance, and/or set up a payment plan.

Are you in my insurance network?

ATI is contracted with many major insurance carriers. View our insurance page for a complete list of in-network insurances accepted. ATI’s billing staff will be happy to verify your health care benefits for you. Please call 855-MY-ATIPT (855-692-8478)for assistance.

What does deductible, co-payment and co-insurance mean?

A deductible is the dollar amount that must be paid by the insured prior to insurance cost-sharing. Patient is responsible for all allowable charges until deductible is met.

A co-payment is the dollar amount the insured is responsible for paying each visit. After the deductible is met, the patient is only responsible for the co-pay. ATI collects co-payment at the time of service for all eligible plan participants unless other arrangements have been made in advance.

Co-insurance is a form of cost sharing. The co-insurance is a percentage of allowable charges the insured is responsible for paying each visit. After the deductible is met, the insurance will cost-share with the patient and will cover the portion of the allowable charges remaining after the patient’s co-insurance. ATI collects a portion of co-insurance at the time of service for all eligible plan participants unless other arrangements have been made in advance.

If I am a physician interested in the benefits of research-based physical therapy for my patients, how do I get more information?

ATI is happy to provide physicians with peer-reviewed and published scientific studies on the benefits of research-based treatment programs. Please email us to request studies and to learn more about evidence-based medicine at ATI.

How can I request patient medical records?

To request patient records, please email our Medical Records Department at mwdocumentation@atipt.com. Please include the Authorization for Access, Use and/or Disclosure of Protected Health Information form (download the PFD in our Medical Records section), and the specific information needed for the request. Requests can also be faxed to 630-759-6185.

Do you offer payment plans? Am I eligible for financial assistance?

In many cases, ATI can offer an interest-free payment plan. Patients may set up a payment plan online at ATIpt.com/paymybill or by calling the Patient Advocate department at 855-MY-ATIPT (855-692-8478).

How do I pay my bill online? What are the benefits?

If you have received a statement:

  • Go to ATIpt.com/paymybill from your computer or any mobile device
  • Select Sign Up for an account and complete the required fields
  • On the Add a Patient screen, enter the required information and the registration code that is located on your statement
