- State tax withholdings - Click Here to obtain tax withholding forms for your state. Fax the document to Payroll at 630-328-1550
- Federal tax withholdings - Click Here to update your federal tax withholdings through the employee self-service portal. As a reminder, your username is your first initial, last name, and last 4 digits of your ssn. Your initial password is Therapy1.

- Direct Deposit -
- Please note that ATI’s payroll team may be able to obtain the direct deposit information on file with your Company prior to the partnership with ATI. Under these circumstances, you will be notified and asked to review the information loaded for accuracy by clicking on the direct deposit link in the employee self-service portal Click Here. In the event ATI’s payroll team is unable to obtain this information in advance and you would like to sign up for direct deposit with ATI, you will need to input your account and routing number under the payroll section in the employee self-service portal Click Here. It is important to note that if your direct deposit information was not pre-loaded and you are entering it for the first time, or should you make any edits to the pre-loaded information, you will receive a live, paper check in the mail to allow for a pre-note with your banking institution. Assuming a successful pre-note, your subsequent paycheck will be deposited directly into the account(s) you entered. As a reminder, your username for the employee self-service portal is your first initial + last name + last 4 digits of your social security number (i.e., jsmith1234), and your initial password is Therapy1. Please review any e-mail communications or documents distributed by ATI’s HR team to learn when you are able to begin accessing the employee self-service portal.
- Missed Clock Sheet - Click Here to print a missed clock sheet, which is to be used if you forget to clock in or out.
- The sheet must be provided to your manager by the end of the pay period so that he/she can enter your missed time into the timekeeping system.
