Baseline Concussion Test Scheduling
ATI has set up convenient times to provide baseline concussion screens for Puyallup School District Athletes. Proof of baseline concussion testing is required prior to participation in Football, Soccer, Basketball, and Wrestling, and are valid for two full years. Baselines are optional for all other sports. Dates will be setup before each sports season.
Baseline concussion testing is by appointment only. Times are set up to accommodate the highest volume of athletes in the most efficient way possible. You can expect the overall time for the baseline to take 15-25 minutes.

**We will NOT be accepting walk-in baselines at this time.**
Please complete the form below to schedule your baseline concussion test, which will take place on the dates and times below at ATI's Puyallup-South Hill clinic, 3912 10th Street SE, Suite 101, Puyallup, WA, 98374.
An ATI Sports Medicine representative will contact you to schedule the test time in 2-3 business days. The fee for baseline concussion testing is $15.00 per person and payable at test time via cash, check or credit card.
- Concussion test scheduling using this form is valid only for members of the Puyallup School District in the state of Washington.
- A pre-participation baseline concussion test is only required for FOOTBALL, SOCCER, BASKETBALL AND WRESTLING For all "Other" sports, the baseline test is optional.
- Test results are archived at ATI Physical Therapy for access when a concussion is suspected.
