PSD Sports Physicals
We apologize but due to high demand the maximum capacity of scheduled sports physicals has been reached. The Puyallup School District hopes to offer complimentary sports physicals in the future but at this time recommends that you reach out to your primary care provider to provide this service. If you need to schedule a baseline concussion test for an athlete participating in Football, Basketball, Soccer and Wrestling, more information can be found at: www.atipt.com/psdconcussion
The Puyallup School District is teaming up with Multicare Health System and ATI Physical Therapy to provide a limited number of complimentary sports physicals for athletes entering the '19-20' school year. All provided physicals meet the WIAA requirements and are valid for 24 months.
What to Expect
Athletes will need to check in to the registration desk 10 mininutes prior to their scheduled appointment time. After that step they will proceed through the physical exams on their own and parents will meet them after steps 4 or 5.
- Step 1: Registration Desk to Review Pre-Physical Paperwork
- Step 2: Vitals Assessment
- Step 3: Muskuloskeletal Screening
- Step 4: Medical Exam with Physician, Nurse or Physician Assistant
- Step 5: (optional) Baseline Concussion Testing if Partcipating in Football, Soccer, Basketball or Wrestling
- Step 6: (optional) Complete the PSD online clearance forms on available computers
After completing the physicals, athletes and families will have access to computers on-site to complete their 2019-20 athletic forms. The entire process from start to finish should take between 30 and 50 minutes.
