Facilitating Patient Transition from Pain to Independence

If you’re homebound following surgery, you may be a candidate for home health. When a physician certifies a patient as homebound, ATI Home Health creates a smooth transition from hospital to home to outpatient therapy. Eligibility requirements include:

  • Physician signs order for services
  • Services needed are reasonable and necessary to treat the illness or injury
  • Leaving home isn’t recommended because of a patient’s condition
  • Patient’s condition keeps him/her from leaving the home without help, such as using a
  • wheelchair or walker, needing special transportation, or getting help from another person
  • Leaving home takes a considerable and taxing effort
